Dear Dr. Proskiw: I have heard that there are new procedures for removing varicose veins from my legs that don\'t involve surgery. Can you explain the difference between the traditional procedure and the newer ones for removing varicose veins?


Answer: Vein stripping has been the traditional surgical procedure for removing varicose veins. It begins with an incision in the groin region. Through this incision the surgeon ties off the saphenous vein and small veins with stitches. Then the surgeon makes a second incision either below the knee or at the ankle and does the same thing on that end. When both ends of the vein are loose the surgeon threads a thin, wire instrument (the stripping tool) through the vein from the groin to the second incision. The surgeon then ties the stripping tool to the vein and pulls or strips the vein from the leg.


Vein stripping used to be standard practice for removing large, bulging varicose veins. With recent medical advances it is not my first choice. Vein stripping requires general or spinal anesthesia and several incisions and is generally performed in a hospital. Following the procedure, there is usually some discomfort and a necessary recovery period.


Question: Can you describe how the newer procedures are performed?


Answer: Lasers are currently used for numerous medical applications throughout the body. Recently, the development of the ELVeS (Endo Laser Vein System), utilizes laser energy to treat varicose vein conditions. The ELVeS system is a minimally invasive procedure where laser energy is delivered to the faulty vein, causing it to close. There are many veins in the leg and after treatment the body simply re-routes blood-flow through other healthy veins.


The procedure takes about 45 minutes and is usually performed in your physician\'s office. No general anesthesia is required, just a local anesthetic. Your physician will insert a thin flexible fiber through a sheath into the faulty vein. The laser light will then be emitted through the fiber. As the thermal laser energy is administered, the vein will close from the inside. Only minimal discomfort, if any, should be experienced.


Question: What kind of follow up treatment will be required after the procedure?


Answer: You will be required to wear a compression stocking for one week after the procedure. Other than that, normal activities can be resumed immediately after ELVeS treatment, with the exception of rigorous exercise.
